ผมร่าง จม. คร่าวๆให้ ไปเติมข้อมูลตรง xxx ด้วยนะ ถ้ามีหลักฐานว่าเราได้ยกเลิกบริการแล้ว (ปกติผมจะ capture หน้อจอ หรือปรินต์เป็น PDF เก็บไว้) ขอให้แนบเมล์ไปด้วยเลยนะครับ ตรงคำว่า "membership no." อาจเปลี่ยนเป็น "reference no." ก็ได้นะครับ (ถ้ามี)
